About Jennifer Jordan

Jennifer Jordan, DNP, APRN, FNP-C, VWCN, is a primary care provider at Jordan Healthcare Center in Southwick, Massachusetts. 

Dr. Jordan earned her Doctor of Nursing Practice (DNP) from Elms College in 2018 and became board certified as a family nurse practitioner. With nearly 22 years of experience in all areas of healthcare, Dr. Jordan believes in a collaborative approach to bringing wellness to the whole person, family, and community. 

Dr. Jordan believes healthcare does not end once symptoms are treated. True wellness is a state of whole-being that’s achieved by building partnerships between patients and providers. 

As a provider, Dr. Jordan welcomes the opportunity to coach and educate her patients, their families, and the community about achieving and maintaining optimum health. Some of her patients even call her their quarterback! 

Dr. Jordan’s approach to medicine is deeply rooted in achieving a healthy lifestyle to promote wellness while meeting her patients where they are in their journey.
