Jordan Healthcare Center is a trauma-informed integrated primary care practice in Southwick, Massachusetts, led by Jennifer Jordan, DNP, APRN, FNP-C, VWCN. As an experienced nurse practitioner, Dr. Jordan practices advanced nursing, focusing on the patient, not their symptoms. 

Dr. Jordan received her Doctor of Nursing Practice (DNP) at Elms College in Chicopee, Massachusetts, and is a board-certified family nurse practitioner. 

Jordan Healthcare Center provides care for patients ages 12 and up, and offers adolescent care, gynecology care, men’s health services, and mental health services. Dr. Jordan also specializes in chronic disease management and has an addiction recovery program.

In addition to providing primary care, the practice also offers aesthetic services, such as platelet-rich plasma (PRP) with microneedling and cosmetic injectables like Botox® and fillers, group classes, and non-pharmacological pain management.
